Chapter 01 · A true story

Even NASA makes mistakes. Really big ones.

In 1999, NASA sent a spacecraft called the Mars Climate Orbiter all the way to Mars. It never arrived. Here’s why.

Two engineering teams worked on the same spacecraft. One team measured pushes in metric units. The other used English units. Nobody noticed the mismatch — so every tiny course correction was a little bit wrong. After 400 million miles of tiny wrongs, the spacecraft flew too close to Mars and was lost.

Two teams used two different measuring systems for the same job — like one friend counting in inches while the other counts in centimeters.

A spacecraft worth hundreds of millions of dollars, and years of work by very smart people — gone in one morning.

What NASA did next

They didn’t hide it. They studied exactly what went wrong, wrote the lesson down, and changed how every future mission double-checks units.

The big idea: smart people and smart machines still make mistakes. What makes them smart is what happens next — they notice, they fix, and they make sure the same mistake never wins twice.
